Application should include the following:

  1. A complete copy of standard application
  2. A personal letter which includes the following:
    • Special Competencies for the position
    • Sources for further background information
    • Pertinent information not included elsewhere
    • Available times for interview
  3. Resume not to exceed two pages
  4. Confidential file forwarded by the candidate's placement office
  5. Copy of credentials
  6. Copies of transcripts
  7. A short statement of education philosophy

Fingerprinting and TB Tests

Federal law requires completion of a fingerprint/background check through the Department of Justice and proof of a negative TB test before your first day of work, whether you are reporting to a substitute or permanent position. Successful completion of these requirements is a condition of employment. The applicant/employee is responsible for all related costs for these processes. If you are applying for a permanent position, you will not be asked to go through the Live Scan process or take a TB test until an official offer of employment is presented to you, but it would still be at your expense.

  • There is a $32.00 fee payable to the Kelseyville Unified School District, prior to issuance of the "Request for Live Scan Service" form. ($32.00 is the amount the Department of Justice charges the district for the processing of each "Live Scan" report.) You must get the form from the Kelseyville Unified School District's Business Office at 4410 Konocti Road in Kelseyville before you submit to the Live Scan process.
  • The Live Scan rolling services are provided by the Lake County Office of Education or the UPS Store.

There is an additional $25.00 fee required at the Lake County Office of Education or UPS Store, for their rolling fee.

  • After completing a Live Scan, return a copy of the completed Live Scan form to the Kelseyville Unified School District's business office. We should receive the results from the Department of Justice in approximately one week. Someone from the district will then notify you of the results.  
  • TB tests can be performed by your regular doctor or by the following:  Lake County Health Department  262-1090, Lakeside Health Clinic 263-7725, or the Upper Lake Clinic 275-9066.

Open the appropriate file and choose print from the toolbar. Please note: the "Release of Information" contract must be signed and returned.
